IMPORTANT DATES FOR 2011-2013 LCTS/RESERVE FUNDS GRANTEES

 Mid-Year Report #1 (July 1 – December 31, 2011):     

 Narrative/Fiscal due Tuesday, January 31, 2012.

 Year-End Report #1 (January 1 – June 30, 2012):      

 Narrative/Fiscal due Tuesday, July 31, 2012.

 Mid-Year Report #2 (July 1 – December 31, 2012):      

 Narrative/Fiscal due Thursday, January 31, 2013.

 Parent Satisfaction Surveys

Must be completed and reported annually. Please include the return rate and summary of the responses. You may use the PACT for Families template, but you must add at least 3 questions that are specific to your program. Per review and approval of PACT for Families, you may also use an agency specific form for collecting parent satisfaction feedback. If this option is selected, please provide us with a copy of your agency satisfaction survey.

 Narrative/Fiscal Reports

Please read the instructions in the report outline carefully and thoroughly complete. The Fiscal Report specific for your grant program will be emailed to you by Shawna Steffen as the due date for the reports approaches.

Please provide thorough reports. Those submitted with missing information or duplicated text from previous reports will be returned.
