~ Important Reminders for LCTS Grantees ~

·        Remember that the Year-End Report for your LCTS grant is due in July. The reports will cover the budget period of January 1, 2008 – June 30, 2008 and will at times ask for brief information covering the entire grant year of July 1, 2007 – June 30, 2008. 

Please note these dates:

Tuesday, July 15, 2008: Type forms are due

Thursday, July 31, 2008: Narrative and Fiscal reports are due

Reports must be turned into the PACT 4 office no later than 4:30 PM on the respective due dates.

All reporting materials are available via the “LCTS Documents” link to the left. If you have any problems accessing the documents, please contact Sarah Wennerberg at sarah_w@co.kandiyohi.mn.us.  

NOTE: Previous versions of any report materials including the Narrative Report, Fiscal Report, or type forms WILL NOT be accepted. Your report will be returned to you if it is not in the current format. 

·        Incomplete reports will result in delayed reimbursement.

·        Take time to review all items in the report form carefully.  Please note the addition to the Narrative Report of the Budget Explanation section (section VI). 

·        As part of your grant contract, Parent Satisfaction Surveys must be completed annually. If you did not provide Parent Satisfaction data on the Mid-Year Report, you must complete the survey process during this report period (January 1 – June 30, 2008) and report the data on the Year-End Report.

·        Grant contact-persons will receive a program-specific Fiscal Report Form from Shawna Steffen via email. This form will have your awarded budget amounts pre-entered.  Please forward on to the appropriate person in your agency to complete this portion of the report. Complete the form for the appropriate budget period.

·        If any contact information for your grant has changed, please notify us.
