REPORT INFORMATION

Type Forms

Please see the list below to see which, if any, type forms you need to complete. Our state-reporting requirements have changed as LCTS guidelines have changed.

Type I v. 2007-1: New Connections Youth Day Trtmt. (Woodland Centers); Non-County Home Based Services; HCBS Intensive In-Home; HCBS Restorative Justice Programs.

Public Health Type form v.2009-1: All Public Health programs (each program need to use a separate form).

NO TYPE II REPORTING REQUIRED: For all other grants. ** Please pay special attention to the modified narrative as the number-served will be reported differently than past years.

Parent Satisfaction Surveys

Must be completed and reported annually. Please include the return rate and summary of the responses. You may use the PACT 4-Created form posted here, but you must add at least 3 questions that are specific to your program. You may also use your agency’s own form for collecting parent satisfaction data.

Narrative/Fiscal Reports

The Narrative outline is posted on this website. Please read the instructions carefully as some sections have change from past years. The Fiscal Report specific for your grant program will be emailed to you by Shawna Steffen as the due date for the reports approaches.
